CharterCARE Health Partners invites Advanced Nurse Practitioners (APRN) and Physician Assistants (PA-C) to apply for the Care at Home Nurse Practitioner (hospital employed) opportunity.  In this role, you will provide comprehensive clinical assessments to four to six patients per day, medical management, and vital health education to both patients and their families in a home setting. You will enjoy work-life balance with a Monday through Friday daytime schedule with exceptionally light shared rotation call coverage.   • Nurse Practitioner (APRN) or Physician Assistant (PA-C) with preferred 2+ years of clinical experience
  • Rhode Island Licensed, Federal DEA, and Controlled Substance Registration
  • Valid Unrestricted Driver’s license and Proof of automobile Insurance
